Brief description: Toast fell on the floor

Detailed Incident Description:

During the standard daily breakfast preparation an incident occurred which resulted in 1 x piece of buttered toast falling onto the floor and subsequently being discarded.How the incident unfolded:An employee (Senior Toaster Operator #1) used the toaster to toast 2 x pieces of sliced Vogel's bread. The operator applied butter to both pieces of toast, and returned the butter to the fridge. During this maneuver the operator bumped the plate containing both pieces of toast with their elbow resulting in one piece of toast falling onto the floor and landing buttered side up. The floor toast was assessed as a health and safety hazard and discarded into the bin. The other piece of toast was unaffected and subsequently eaten.
